Medical Visa from Nigeria to Poland Description

This Medical Visa is issued to citizens of Nigeria who are traveling to Poland to seek and receive medical treatment.
A medical report from the applicant’s local physician should be provided. The report should state the treatment needed by the applicant and the reason the treatment will be best gotten in the country.
A letter from the receiving medical institution in Poland stating willingness to treat the applicant is required.
Any other correspondence between the receiving and sending medical institution should be provided.
Proof that the applicant can pay the medical fee and any other extra fee that may arise is required.
Minors applying for this visa are required to be in possession of the original copy of their birth certificate.
You are expected to provide a letter from your employer confirming awareness and allowance of your business leave.
Submitting fake or forged document will result in you being denied this visa. You are also likely to be prosecuted for this.

Passport with a validity of at least 3 months from the intended period of exiting the country. The passport should have at least two blank pages for the necessary stamp.
Duly completed visa application form signed by the applicant.
Two recent passport-sized photographs with a white background. The photo should follow the specification for a visa application.
Original copy of bank statement from the past three months prior to the period of visa application. This is proof of a steady and continuous flow of funds.
Travel insurance which has sufficient medical coverage. The insurance should be valid for the duration of your stay in the country.
Proof of civil status.
Proof of accommodation for the period you will be staying in the country.
Copy of return flight ticket.
